The increasing use of infusion pumps in clinical practice has brought convenience to our work. If they are not maintained or used properly, they will affect patient safety and cause damage to the instrument. So how much do we know about the use of infusion pumps?
1. Daily maintenance
Regular maintenance of the batteries in use. Avoid the battery liquid overflow caused by not replacing the battery for a long time, corrode the power supply circuit board, and cause unnecessary losses; regularly check whether the battery contacts are corroded. If there is any problem, use a soft eraser to remove it.
When the injection pump is in use, other machines may be hung around, such as nutrition pumps, infusion pumps, etc. When the liquid discharged from other machines accidentally drips onto the injection pump, it should be wiped clean in time. Because the liquid seeps into the inside of the machine, it may cause damage to the circuit board. In addition, glucose and other liquids will cause adhesion between pump pieces, resulting in excessive resistance when the pump piece is squeezed, affecting the accuracy of infusion, so it is necessary to pay attention to keep the surface of the injection pump clean.
The fixed bracket and locking rod head of the injection pump are hard plastic products, which are easy to break, so when storing the injection pump, please put the injection pump in a cool place that is not easy to touch. Place the syringe pump correctly and keep the parts in place.
2. Appearance inspection before use
① The appearance of the equipment shall be clean and tidy, the surface of the pump body shall be clean, the injection pump tank shall be free of stains, the door of the infusion pump shall be free of stains, and the sensor part shall be free of stains.
② The indication of external power supply and internal power supply (power supply battery) of the equipment shall be normal.
③ All parts of the pump (box, power cord, pull bolt, rack rod, clamp, infusion pump door, etc.) are in good condition.
All switches and buttons should be effective and sensitive.
⑤ In the daily illumination or light, the screen display content should be clearly distinguishable.
⑥ The warning signs are clear, and the alarm function works normally. In addition to the indicator light or screen display, it should be accompanied by a sound alarm.
3. Pump start delay phenomenon
Some researchers have found that there is a delay in starting the syringe pump. At a speed of 1 mL/h, the delay time of different syringe pumps varies from (6.75±4.40)min to (57.20±28.60)min, which is related to the design of the syringe pump, syringe specifications, injection speed, pump tube compliance and other factors.
4, the use of the process to pay attention to the problem
① The infusion equipment should use special supporting consumables. Different brands of pipelines have different parameters such as hardness and pipeline wall thickness. If they do not match the parameters of the infusion pump, the infusion rate will be inaccurate.
② When the infusion speed is slow and the puncture site is higher than the pump body, venous reflux often occurs, which is easy to cause needle blockage and affect the treatment. When the injection pipeline is blocked, the micro pump will not immediately alarm, only when the pipeline is blocked for a certain period of time.
When the input speed of micro-pump is lower than 5 mL/h, it is easy to block the tube. Normal saline or glucose solution must be input at the same time. Maintaining at the speed of 5 drops/min will not cause adverse reactions, nor will it increase the heart load, which can effectively prevent blood return.
③The main factors to avoid non-human leakage of bubbles: liquid medicine is usually stored at low temperature. Due to the high gas solubility of low-temperature liquid, when infusion in the ward, with the temperature of liquid medicine gradually increasing, its gas solubility decreases, and tiny bubbles will be resolved. Liquid ingredients, such as cardiovascular and cerebrovascular drugs, traditional Chinese medicine, etc., will also produce bubbles.
④ The distance between the Maofei dropper of the infusion set and the entrance above the infusion pump door should be kept> 10cm, not too short or too long, otherwise it will affect the accuracy of the infusion. The fatigue degree of the infusion pipeline used by the infusion pump changes with time. In view of the accuracy and safety of the infusion pump manufacturers in the case of domestic supporting consumables, the extrusion at the same position cannot exceed 4 hours when using ordinary infusion sets. It is recommended that the infusion set should be pulled down for a period of time every 4 hours or so.
⑤ The drop number sensor should be installed 1 cm above the liquid level of the Mauffeured tube. The Mauffeured tube should be kept vertical and the drops can pass through the sensor vertically.
⑥ Beware of drug extravasation! In the process of bolus injection, such as drug extravasation, infusion pump alarm system will not respond, if not actively take positive and correct measures, will have serious consequences.
⑦ Pump speed adjustment error: because the operator is not familiar with the speed setting key, or the speed is not changed in time after changing the drug, or in some cases, the speed setting is accidentally touched by others, which changes the speed, causing the drug to enter the body too much or not enough, resulting in adverse consequences.
How many bubbles are safe? After the bubble rate <0.02mL/(kg/min) enters the human body, it can be measured, but it does not cause physiological disorders. When the bubble rate into the human body is> 0.4mL /(kg/min), the pulse pressure may increase. The lethal dose of continuous entry of bubbles is> 2mL/(kg/min); That is, a person weighing 50kg can be killed by inputting 100mL bubbles within 1 minute.
